Where does the special domains name like .FM, .LA, & .TV come from?

The special domain names are called ccTLDs (Country Code based Top Level Domains) and are assigned to a specific country that allows for marketing of the TLD. As an example the .FM domain extension is assigned to the Federated States of Micronesia and is made available through dotFM.
